Blocked ears are miserable — the muffled hearing, the sense of fullness, the occasional ringing. And yet it's a problem that many people in Barnsley and South Yorkshire put up with for months, partly because they're unsure where to go, and partly because NHS ear syringing has largely been withdrawn from GP surgeries across England.

Why can't I get ear syringing at my GP any more?

In recent years, the majority of GP surgeries in England — including many in Barnsley — have stopped offering ear irrigation (syringing) as a routine service. NICE guideline NG221 (Earwax: assessment and management, 2023) now recommends that patients with troublesome earwax should be offered ear irrigation or microsuction, but the service has been largely removed from NHS primary care contracts — leaving many patients with no obvious NHS pathway, long waits at audiology clinics, or paying for private treatment.

Microsuction vs irrigation: what's the difference?

Microsuction

Microsuction uses a gentle suction device to remove earwax under direct visualisation. Your pharmacist uses surgical loupes (magnifying lenses) to see exactly what's happening in the ear canal throughout the procedure. The wax is never pushed deeper, and no water is used. NICE Clinical Knowledge Summary on earwax describes microsuction as the preferred method where available due to its safety profile and precision.

Irrigation (ear syringing)

Irrigation uses a pressurised jet of warm water to flush wax out of the ear canal. When performed correctly, it can be effective, but it carries a slightly higher risk of discomfort, infection, and — if wax is very hard — pushing it further into the canal. It's also not suitable for patients with perforated eardrums or a history of ear surgery.

Which should I choose?

At Brampton Village Pharmacy, we use microsuction as our primary technique. It's safer, more comfortable, and more precise. Irrigation is available as an alternative in specific situations. Your pharmacist will assess which is most appropriate during the appointment.

How to prepare for earwax removal

We strongly recommend using olive oil ear drops twice daily for 3–5 days before your appointment. This softens the wax and makes removal significantly easier and more comfortable. NICE CKS guidance on earwax management supports the use of olive oil or sodium bicarbonate drops as a softening agent prior to removal. Olive oil drops are available from us or any pharmacy.
