Erectile Dysfunction: Common, Undertreated, and Reversible

Erectile dysfunction affects approximately half of all men aged 40–70 to some degree, and around 10% of all adult men have complete ED. Despite this prevalence, it remains significantly under-discussed and under-treated — particularly in South Yorkshire communities where cultural attitudes to men's health traditionally discourage seeking help for sensitive conditions. Many men experiencing ED suffer in silence for months or years.

ED is often both a symptom and a risk marker. In younger men (under 40), psychological factors including performance anxiety, stress, relationship difficulties, and depression are common causes. In older men, vascular disease — reduced blood flow to the penis — is the most frequent physical cause, and ED can be an early warning sign of wider cardiovascular disease. Men with type 2 diabetes, hypertension, high cholesterol, or who smoke are at elevated risk due to vascular and nerve damage.

Effective pharmacological treatment for ED has existed since 1998. These treatments work for the vast majority of men with ED of vascular origin, are taken on demand or daily, and have a well-established safety profile. Sildenafil vs Tadalafil: Which Is Right for You?

Both sildenafil and tadalafil are phosphodiesterase type 5 (PDE5) inhibitors — they increase blood flow to the penis in response to sexual stimulation. They do not create an erection independently; sexual arousal is still necessary. The main differences are in duration, timing flexibility, and dosing options.

Sildenafil (the active ingredient in Viagra) is taken 30–60 minutes before anticipated activity and lasts 4–6 hours. A fatty meal before taking sildenafil can delay its onset. It is available in doses of 25mg, 50mg, and 100mg — most men start at 50mg. It is highly effective (60–70% response rate across all ED severities) and available at significantly lower cost as a generic.

Tadalafil (the active ingredient in Cialis) lasts up to 36 hours — removing the need to time the pill closely before activity, which many men find less clinical. It can also be taken as a daily low dose (5mg), providing a continuous background effect throughout the day. Tadalafil is less affected by food intake. Your pharmacist will discuss both options and recommend the most appropriate starting point for your circumstances.

Hair Loss: Separating Evidence from Marketing

Male pattern hair loss (androgenetic alopecia) affects approximately 50% of men by age 50 and is driven by the action of dihydrotestosterone (DHT) on genetically sensitive hair follicles. The market for hair loss treatments is awash with unproven products, but only two pharmacological treatments have strong clinical evidence: minoxidil (topical) and finasteride (oral).

Minoxidil prolongs the growth phase of hair follicles and increases blood flow to the scalp. It is available without prescription in 2% and 5% solutions and foam. Consistent daily use for at least 4–6 months is required before results are apparent. Response rates are moderate — approximately 40–50% of users experience meaningful regrowth.

Finasteride (1mg daily) blocks 5-alpha reductase, the enzyme that converts testosterone to DHT. In clinical trials, finasteride halted hair loss in over 80% of men and produced visible regrowth in approximately 65%. It requires a prescription, which our pharmacist independent prescriber can issue. Side effects — reduced libido and mild sexual function changes — occur in a small minority of men. Why Men's Health Often Goes Untreated — and Why the Pharmacy Changes This

Research consistently shows that men are significantly less likely than women to seek healthcare for sensitive or embarrassing conditions. The traditional GP appointment model presents multiple obstacles: explaining a sensitive issue to a receptionist, waiting for an appointment slot, sitting in a waiting room, and discussing the problem face-to-face in a time-pressured consultation.
