Mounjaro (Tirzepatide)
GLP-1 / GIP Dual Receptor Agonist · Eli Lilly
How It Works
Mounjaro works on two hunger-regulating hormones simultaneously — GLP-1 and GIP — making it the first dual-action weight loss injection. It reduces appetite dramatically, slows the rate at which food leaves your stomach, and helps your body use insulin more effectively. Because it activates two pathways rather than one, clinical trials show significantly greater weight loss than single-action GLP-1 drugs.
How to Take It
Once-weekly self-injection using a pre-filled auto-injector pen. Injected into the abdomen, thigh, or upper arm. Most patients manage this confidently after a brief demonstration from our pharmacist.
Clinical Results
In the SURMOUNT clinical trials, patients lost an average of 20–22% of their body weight over 72 weeks — the highest recorded weight loss of any approved medication. In real-world use, a 15–20 kg weight loss over 12 months is typical for patients who remain on treatment.
Eligibility
BMI 30+ (or BMI 27.5+ with a weight-related condition such as type 2 diabetes, high blood pressure, or high cholesterol). Not suitable during pregnancy, with a personal or family history of medullary thyroid carcinoma, or with pancreatitis. Full clinical assessment at your first appointment.

Wegovy (Semaglutide)
GLP-1 Receptor Agonist · Novo Nordisk
How It Works
Wegovy contains semaglutide — the same active ingredient as the diabetes medication Ozempic, but at a higher dose specifically licensed for weight management. It mimics the GLP-1 hormone released after eating, telling the brain you are full and reducing appetite. It also slows gastric emptying, so you feel satisfied for longer after smaller meals.
How to Take It
Once-weekly self-injection using a pre-filled auto-injector pen. Injected into the abdomen, thigh, or upper arm. The dose is gradually increased over 16–20 weeks to minimise side effects such as nausea.
Clinical Results
In the STEP clinical trials, patients lost an average of 14.9% of their body weight over 68 weeks. In practice, most patients lose 10–15% of body weight over 12 months. Wegovy is also the first weight loss medication to demonstrate a statistically significant reduction in cardiovascular events (heart attacks and strokes) in overweight patients.
Eligibility
BMI 30+ (or BMI 27.5+ with a weight-related condition). Not suitable during pregnancy, with a personal or family history of medullary thyroid carcinoma, Multiple Endocrine Neoplasia syndrome type 2, or with pancreatitis. Full assessment at first appointment.

Orlistat (Xenical / Alli)
Lipase Inhibitor (Oral Capsule) · Various (generic available)
How It Works
Orlistat works differently to injectable treatments — it does not affect appetite or hormones. Instead, it blocks approximately one-third of the fat you eat from being absorbed in your gut. The unabsorbed fat passes through your digestive system and is excreted. This means you absorb fewer calories from every meal. It is most effective when combined with a reduced-calorie, lower-fat diet.
How to Take It
Oral capsule taken three times daily with each main meal containing fat. If a meal is skipped or contains no fat, the dose for that meal can be omitted. Available as prescription-strength 120 mg (Xenical) and over-the-counter 60 mg (Alli).
Clinical Results
Clinical trials show an average weight loss of 5–7% of body weight over 12 months when combined with dietary changes — less than injectable treatments but with a well-established safety profile over many years of use. Orlistat is a suitable option for patients who prefer an oral medication or who do not meet the eligibility criteria for injectable treatments.
Eligibility
BMI 28+ (with a risk factor such as diabetes or high blood pressure) or BMI 30+. Requires commitment to a low-fat diet to minimise gastrointestinal side effects. Not suitable during pregnancy or with chronic malabsorption syndrome. Full clinical review at first appointment.

Mysimba (Naltrexone / Bupropion)
Centrally Acting Oral Tablet · Orexigen / Nalpropion Pharmaceuticals
How It Works
Mysimba combines two medicines — naltrexone (used in addiction treatment) and bupropion (an antidepressant and stop-smoking aid). Together, they act on two areas of the brain involved in hunger, reward and food cravings. Bupropion activates pro-opiomelanocortin (POMC) neurons that suppress appetite; naltrexone blocks the feedback loop that would normally switch them off. The result is a sustained reduction in hunger and food cravings.
How to Take It
Oral tablet taken twice daily. The dose is started low (one tablet in the morning for week 1) and increased gradually over four weeks to the full dose of two tablets twice daily. Should be taken with food to reduce nausea.
Clinical Results
In the COR clinical trials, patients lost an average of 6–9% of body weight over 56 weeks. Mysimba is particularly effective in patients with strong food cravings or emotional eating patterns, and in those where appetite-focused treatments have not been sufficient alone.
Eligibility
BMI 30+ (or 27+ with a weight-related condition). Not suitable for patients with uncontrolled high blood pressure, seizure disorders, eating disorders (anorexia, bulimia), those currently taking opioids or opioid-containing medications, or during pregnancy. Full clinical assessment required.

Why Choose a Pharmacist-Led Weight Loss Clinic?
NHS weight management services in Barnsley and South Yorkshire face significant demand pressures. Specialist tier-3 weight management programmes often carry waiting times of twelve to eighteen months, and many patients are turned away at GP level because they don't yet meet the threshold for referral. For people living with obesity-related conditions such as type 2 diabetes, high blood pressure, or sleep apnoea, that wait is not just inconvenient — it is clinically harmful.
Brampton Village Pharmacy's weight loss clinic bridges this gap directly. Our lead pharmacist is an independent prescriber with extensive primary care experience across South Yorkshire, meaning we can complete a full clinical assessment and issue a prescription for Mounjaro, Wegovy, Orlistat, or Mysimba in a single appointment — without requiring a GP referral or a secondary-care waiting list. Every treatment plan is individually tailored and backed by monthly clinical monitoring.
The pharmacy model also removes many of the barriers that prevent people from seeking help. Appointments run during opening hours including Saturday mornings, there is no lengthy intake form or multi-stage referral, and the consultation room is private and confidential. For patients in Goldthorpe, Wombwell, or Mexborough — where access to specialist services is particularly limited — this represents a genuine alternative to waiting.
Our initial weight loss consultation is free of charge. You are not committed to any treatment plan until you have had a chance to discuss your options fully with the pharmacist.
The Clinical Assessment: What Happens at Your First Appointment
Your first appointment at our weight loss clinic typically takes 30–45 minutes. The pharmacist will begin with a structured clinical assessment covering your medical history, current medications, any known allergies, and relevant family history. This is essential — some weight loss medications have contraindications with common drugs such as insulin, sulphonylureas, and certain blood pressure medications, and these interactions must be identified before any prescription is issued.
You will have your weight, height, and BMI measured, and your blood pressure checked using a calibrated digital monitor. For patients starting injectable treatments such as Mounjaro or Wegovy, the pharmacist will also screen for a personal or family history of thyroid cancer or multiple endocrine neoplasia syndrome type 2 (MEN2), as these are absolute contraindications. A brief cardiovascular risk assessment is also completed.
If treatment is clinically appropriate, a prescription is issued on the day and the medication can often be dispensed immediately from our in-pharmacy stock. You will receive a full written treatment plan, detailed instructions on administration, and a schedule for monthly review appointments. Your GP will be notified that you have started treatment, ensuring your primary care record is kept up to date.
Bring a list of any current medications — including supplements — to your appointment. This helps the pharmacist complete your medicines reconciliation quickly and accurately.
Pharmacist vs GP for Weight Management: When to See Who
Our weight loss clinic is appropriate for adults aged 18 and over with a BMI of 30 or above (or 27.5+ for patients of South Asian heritage, reflecting higher cardiometabolic risk at lower BMI thresholds), who do not have contraindications to the proposed treatment. We can manage the complete treatment pathway — from initial assessment and prescription through to dose titration and ongoing monitoring — without any GP involvement required.
There are circumstances where GP or specialist input is advisable alongside pharmacy-led treatment. If you have complex, unstable type 2 diabetes managed with insulin or multiple oral agents, your GP should be aware of any changes to your weight management plan as significant weight loss can alter your insulin requirements. Similarly, patients with active eating disorders, chronic kidney disease stage 4 or above, or a recent history of pancreatitis should discuss weight management with their GP before starting injectable treatments.
If you are unsure whether you need GP involvement, simply attend your free initial consultation at the pharmacy. Our pharmacist will give you a clear, honest assessment of whether our clinic is suitable for you, or whether an alternative pathway would serve you better. We never proceed with treatment where clinical uncertainty exists.
Weight Loss Goals and Seasonal Motivation: Making It Stick
January is consistently the pharmacy's busiest month for weight loss enquiries — and for good reason. The new year brings renewed motivation, and clinically supervised weight loss with modern GLP-1 medications can achieve results that are genuinely transformative. Research published in The Lancet shows that tirzepatide (Mounjaro) can produce mean weight loss of over 20% of body weight in patients without diabetes — results that were previously only achievable through bariatric surgery.
However, sustainable weight management requires more than a seasonal burst of motivation. Our clinic is designed for year-round support, not a crash-diet approach. Monthly review appointments allow dose titration based on your response and tolerance, monitoring for side effects such as nausea or gastrointestinal symptoms (common in the first 4–8 weeks), and ongoing lifestyle coaching. Patients who engage with the full programme — including the dietary and physical activity guidance we provide — consistently achieve better long-term outcomes.
Summer is also a high-demand period as patients prepare for holidays or return from travel health consultations wanting to address their wider health. Whatever the season, we encourage anyone considering weight management to start the conversation early rather than waiting for a "perfect time" — the best time to act is always now.
Side effects from GLP-1 medications like Mounjaro are usually strongest in the first 4 weeks and improve significantly as your body adjusts to the medication. Starting on a Friday can help if you want to manage initial side effects over a weekend.
