How It Works

Hospital Referral

When you are discharged from hospital with new or changed medicines, the hospital team can refer you to the Discharge Medicines Service at your nominated pharmacy. This is triggered automatically at participating hospitals.

Pharmacist Follow-Up

Your pharmacist contacts you (or your carer) within a few days of discharge to arrange a consultation. This can be in person at the pharmacy, by phone, or at home if mobility is limited.

Medication Review

Your pharmacist checks you understand all your new medicines, identifies any discrepancies between what was prescribed in hospital and what you've been given, and answers your questions.

GP Liaison

Any concerns or discrepancies are communicated directly to your GP. A full written summary of the consultation is shared with your GP practice to ensure continuity of care.
