How It Works
Initial Quit Consultation
A 30-minute consultation to discuss your smoking history, previous quit attempts, triggers, and to measure your carbon monoxide level. Together you set a quit date and agree the most appropriate NRT or medication.
Quit Date
You start your quit attempt on the agreed date, supported by your chosen NRT or Varenicline. The first two weeks are the hardest — weekly check-ins begin.
Weekly Follow-Ups (12 weeks)
Regular short appointments to check your CO level (confirming you've quit or measuring progress), discuss cravings and triggers, adjust NRT dose, and provide motivational support.
Completion and Relapse Prevention
At 12 weeks, if you've quit successfully, you'll receive guidance on long-term maintenance, managing high-risk situations, and what to do if you slip.

It works through a dual mechanism: as a partial agonist at nicotinic acetylcholine receptors, it partially satisfies nicotine cravings while blocking the receptors from responding fully to nicotine — reducing the reward of smoking and making cigarettes less satisfying. Clinical trials show varenicline approximately doubles quit rates compared to nicotine replacement therapy alone.
Varenicline is taken orally, starting one to two weeks before your quit date. The initial dose is 0.5mg daily for three days, then 0.5mg twice daily for four days, then 1mg twice daily for the remaining 12-week course. Common side effects include nausea (most common, usually mild and settles over time), vivid dreams, and headache. Most patients tolerate it well.
Varenicline is available on NHS prescription through our pharmacy. Your pharmacist will assess suitability — it is not recommended in pregnancy or for certain kidney conditions — and issue the prescription if appropriate. Subsequent prescriptions can be managed directly through our repeat prescription service.

Nicotine Replacement Therapy: Choosing the Right Products
NRT works by delivering controlled, reduced doses of nicotine without the harmful chemicals in cigarette smoke — bridging the gap between stopping smoking and achieving full nicotine independence. Many people who use NRT but still struggle to quit are under-dosing: using too low a strength or not replacing enough of their cigarettes' nicotine. Your pharmacist will ensure your dose is matched to your actual cigarette consumption.
The main NRT options are patches (slow, steady dose over 16 or 24 hours), gum (faster release for acute cravings, works in 5–10 minutes), lozenges (similar to gum, dissolve in the mouth), inhalators (plastic tube delivering nicotine through the mouth), and nasal or mouth sprays (fastest-acting, useful for strong breakthrough cravings). Combining a patch for background nicotine replacement with a fast-acting product for cravings is recommended in current NICE guidance and is significantly more effective than using one product alone.
The service is free for all NHS-eligible patients — which includes virtually all smokers. The cost of free NRT provision is substantially less than the long-term NHS cost of treating smoking-related disease.
What to Expect When You Quit: A Recovery Timeline
Understanding what happens to your body when you stop smoking can significantly strengthen your commitment. Within 20 minutes of your last cigarette, heart rate and blood pressure begin dropping towards normal. Within 12 hours, carbon monoxide levels fall to normal and oxygen levels increase. After 24–48 hours, cravings and withdrawal symptoms typically peak before gradually declining over the following weeks.
Withdrawal symptoms are a normal physiological response to nicotine removal and typically include irritability, difficulty concentrating, strong cravings, increased appetite, headache, and disturbed sleep. These are most intense in the first 3–7 days and generally resolve within 2–4 weeks. Both NRT and Varenicline significantly reduce their intensity.
After one year of not smoking, your risk of heart disease is half that of a smoker. After ten years, your risk of lung cancer is half. After fifteen years, your cardiovascular risk is equivalent to a lifelong non-smoker. These are not abstract statistics — they represent real protection from the most common causes of premature death in South Yorkshire.
